Use this form to request an appointment with us. We will reach out to you via email, text or phone call to confirm your appointment, or help you find another date if your interested time slot is not available.
Cancellation Policy: To avoid a cancellation fee of $50, please call or text us at (714) 617-2296 at least 24 hours in advance to cancel your appointment.
LATE POLICY: If you are running late to your appointment, please let us know. If you arrive 15 minutes late or more to your appointment, you might need to reschedule the appointment to another day.
PLEASE NOTIFY US IF YOU ARE NOT FEELING WELL, OR HAVING COLD/FLU LIKE SYMPTOMS, OR HAVE BEEN EXPOSED TO SOMEONE WITH COVID-19, YOUR APPOINTMENT WILL BE RESCHEDULED TO A LATER DATE.
